Goody Bag Stuffers Request Form
Please follow the steps below:
| Step 1: | Allow at least 60 days for processing and shipping via UPS Ground.

| Step 2: | Be sure you have a street address for delivery within the contiguous 48 States
(no P.O. Boxes).

| Step 3: | Download and print the
Overview & Application Form.
|
| Step 4: | Be sure you complete all information and select the items in order of preference
(if requesting multiple items).
|
| Step 5: | Make a copy for your records.
|
| Step 6: | Submit to Mothers the original completed and signed Application Form and one copy of your Event Flyer
and other supporting materials, if applicable.
|
| Step 7: | If you completed all steps as asked, simply wait until your shipment arrives.
